Need some advice sharpish!

My bike refuses to get into top gear (smallest cog on the back). It does when not under load, but when you're putting power down it just sits there making a bloody racket!
It doesn't even get into gear with the cable completely loosened. So its not the cable.
What else could it be?
Pls help!
